 Documents Needed for Enrolling:

• Withdrawal Form from last school
• Transcript(s) from previous schools
• Birth Certificate
• Immunization Records
• Current IEP/504 Plan (if applicable)  

Proof of Residency; We accept any of the following:

• AZ driver's license or AZ ID or motor vehicle registration 
• Real estate deed or mortgage documents
• Property tax bill
• Residential lease or rental agreement
• Water, electric, gas, cable or phone bill
• W-2 wage statement
• Payroll stub
• Certificate of tribal enrollment or ID issued by Indian tribe that contains AZ address
• Documentation from a state or federal government (Social Security Administration) 
• Veteran's Administration (Arizona Department of Economic Security
